FIT4MOM ROSWELL ALPHARETTA PERIMETER Body Well® Class Pass - 16 Classes AGREEMENT
This contract is made between the client and FIT4MOM Roswell Alpharetta Perimeter and is a pre-paid pass. Payment is required before attending class.
AUTHORIZATION FOR
PAYMENT
I authorize FIT4MOM Roswell Alpharetta Perimeter to initiate a one-time charge to the
account entered into Pike13 for payment.
This Pass is
non-refundable and non-transferrable.
This Pass is to be used between September 11, 2021 and November 6, 2021.
AGREEMENT
By purchasing this pass, you are agreeing to the terms of this contract. Terms
of this agreement may change with proper advance written notice.
